In this test, candidates will meet 12 standardized patients to test their communication and recording skills. Candidates need to communicate with patients professionally, quickly and understanding, answer their questions and take notes. In order to match successfully, IMG needs to increase their own advantages as much as possible: for example, the average score is higher than the AMG, and the recommendation letter proving that the candidates have clinical high-quality experience in American hospitals (must be American hospitals, local hospitals are not recognized), and Hongjing International Education also provides clinical internship services in American hospitals.

This tests whether candidates master the application ability of basic science and their understanding of biomedical and clinical sciences.

The examination includes computer-based case simulation (CCS), which includes assessment of medical history and physical examination information, observation of candidates' ability to select initial treatment and manage patients.
